How should I keep a log of symptoms?

I have a lot of trouble sleeping, but I don't know a whole lot about sleep disorders. I'm not sure if I just have insomnia, or if there might be something more serious at work, like sleep apnea. I want to bring a log of symptoms to a doctor so that I can be treated as effectively as possible, but I don't know what to include. What should I put in my log of symptoms? How should I keep the log?

A daily log could start with how you feel upon awakening and if you are sleepy during the day. You should note the time and length of sleepiness and naps during the day. You should ask your bed partner if applicable, to note if you are snoring and gasping for air while sleeping. Also, keep up with the time you go to bed, get up for the day, number of times you awaken and what woke you up.
